To make a sample request, visit the Sample Request Page and navigate to the Merck Sample Portal. If you are a first-time user of the Merck Sample Portal, you will need to register for an account using your NPI (National Provider Identifier).

Available samples are shipped as soon as the appropriate authorization has been received. Eligible, validated MDs, DOs, and other licensed practitioners (depending on state-level requirements) can expect to receive samples within 3–5 business days of Merck receiving the signed and dated Sample Request Form.

When you register for Merck Connect, you will need to verify that you are a licensed US health care professional, which allows you to access services such as product sample requests. You may verify your license using your state license number, your American Medical Association (AMA) Medical Education number, or your American Osteopathic Association (AOA) number.
